The man was arrested and charged today, and is due to appear in Court at a later date.
Police have charged a 49-year-old Jarrahdale man with 12 offences following an investigation by the WA Police Force Major Fraud Squad.
Authorities charged him with five counts of utter a forged document and one count of attempted fraud, which they allege relate to materials and information that the man allegedly provided during applications involving WA Police Force.
The man has been charged with a further five counts of false evidence before a parliamentary committee and one count of a forged record, which authorities said relate to information provided to a State Parliamentary committee.
The maximum penalty for these offences is seven years imprisonment. He'll front court at a later date.
